ATTENTION SPRING ATHLETES
The Athletic Department is now accepting physicals for spring sports. All physical paperwork MUST be completed using the PIAA forms which can be found on our website.
If you have already had a physical done for a fall or winter sport, or are having your physical done by your family physician or at an Urgent Care facility, please refer to the table below to determine what paperwork is needed.
**We are not permitted to accept wellness visit/driver’s permit physical/etc**

PIAA physical date (initial physical) and Paperwork needed
prior to June 1, 2018 — Not acceptable. We will need a new, complete sports physical (Sections 1-6)
June 1, 2018 to January 21, 2019 — Recertification form (Section 7) signed byparent/guardian. You will also need Section 8 or a physician’s clearance note if you report any injuries in Section 7.
After January 21, 2019 — No additional paperwork required
All physical paperwork for spring sports is due by February 25, 2019 so that it can be processed by the first day of practice on March 4, 2019.
NEED A PHYSICAL?
We will be offering physicals at the high school on February 21, 2019 from 5:45 – 6:45 pm for spring athletes. If you are attending our physicals there are a couple of things you will need to know:
- Our new health care provider, Tower Health Medical Group, requires that we collect Sections 1-5 of the physicals for review no later than February 13, 2019.
- When you arrive for your physical, we will return Sections 1-5 to you and supply you with Section 6 which the doctor will complete.
- There will be no walk-ins permitted. Only those who have completed and turned in Sections 1-5 for review will be given a physical
- There will be a $25 fee for the physical
