Transcript Requests

For transcript request instructions, please select your enrollment status:

Students currently enrolled at the high school should submit the following forms to complete the transcript request process:

  • Transcript Information Form – This form must be completed only once, giving permission to release your transcript.
  • Transcript Release Form –  This form should be submitted for any school that you apply where a transcript is requested.
  • Autobiographical Sketch/Resume – If a school requests a counselor recommendation, please fill out this form and submit to your guidance counselor. If you are applying via the Common Application, this form is mandatory as counselors are required to submit a letter of recommendation.

The guidance office will send up to five (5) transcripts free of charge. There is a charge of $5.00 per transcript for additional transcripts to be sent. Transcripts will be sent within ten school days that the request is received in Guidance
